Mpox (Monkeypox) vaccines
We now provide the Mpox vaccination to existing patients. SA Health is working closely with the local community to help combat the global Mpox outbreak, provide up-to-date information and roll out vaccines to those most at risk.
checkViewport(), 100)"> Where to find us
We are located in the southwest corner of Adelaide city centre at:
17 O’Brien Street
Adelaide, SA 5000
We have no on-site parking but car parking can be found nearby on Gilbert Street.
If you're travelling by bus from the city, the bus to catch for closest access with the most frequency is G10.
This bus travels down King William Street, through Victoria Square and along Grote Street. It then travels along Morphett Street and around Whitmore Square and then heads along Sturt Street. The stop closest to us is Y1, which is located outside Sturt Street Primary School.
Alight from the bus and walk towards West Terrace and O’Brien Street is on the left.
checkViewport(), 100)"> Your medical records
If you are moving to a new medical practice from O'Brien Street Practice for your care, you can request your medical records be moved as well, so your new GP or specialist can access them for continuity of care.
There are two different ways for your medical records to be moved, either in hard copy form or via electronic transfer. Both of these options are secure and confidential.
If you are requesting a copy of your medical records for yourself, you must do this through the CALHN Freedom of Information (FOI) team.
If you require any assistance with your transfer request, contact the practice nurse at O'Brien Street Practice for support and assistance.
checkViewport(), 100)"> How to move or request your medical records
- The new practice must submit a request to us via email, on their own practice letterhead, asking for the named patient medical records to be moved or a health summary to be provided. Also providing preferred format information, either hard copy or electronic transfer.
- The named patient must also complete, sign and return to us a Consent to Release Information form, to accompany the new practice request. You can use one that we have provided or if your new practice is RACGP accredited, they can also provide one to you
- Once both forms are submitted to us, medical records or a health summary will be sent securely to your new practice
- If you have requested an electronic transfer, your medical records will be sent via ShareFile, a secure online platform. As per CALHN Policy, we cannot transfer XML files onto a portable storage devices, including USB drives
- If you have requested a hard copy transfer, medical records will be couriered to your new practice
- Allow 1-2 weeks for your request to be processed.
If, as a part of requesting your medical records be moved, you would also like a copy of your medical records to keep, you can request these through the CALHN Freedom of Information (FOI) team.
It is not necessary to use FOI to have your medical records moved.
To obtain a copy of your medical records, you will need to complete a separate FOI application, which typically incurs a fee.
However, we are currently supporting a fee waiver for all O'Brien Street Practice patients to receive their medical records, if they would like a copy, as they transition to another practice.
If you require any assistance with lodging an FOI request contact the practice nurse at O'Brien Street Practice for support and assistance.
